Appointments for Separations-Final Out can be made 30 days prior to the start of Service Member’s leave, if Service Member is not taking a leave 30 days prior to ETS date. Need to show your leave form (If taking a leave) and orders to make an appointment.

- Installation clearing papers (DA Form 137-2)
- DA 31 (leave form) if taking leave
- DD 2648 Preseparation checklist from ACAP (Bldg 1118)
- DD 93 & SGLI (Updated)
- ERB (Updated)
- Finance request memo
- Light green VA clain receipt (If VA received your medical record)
